Instructions: On a scale of 1-10, assess your current level of skill at the safety practice described in each statement. Type the number into the box next to each statement.

On a scale from 0 to 10, are we reaching out to our relative to the best of our ability considering his/her current condition?

What makes us think that not reaching out creates problems?

What do we think will happen if we don't make a change?

What would be the advantages of making a change? If we scored a 10 what would that look like?

What do we think would work for us if we decided to change?
